What Happens When You Come to Crossroads CSB Same Day Access?
Youth and adults can come in person, without a prior appointment, to be screened for services at the Crossroads Same Day Access (SDA) program. The program operates at two locations:
- Farmville: Tuesdays, Wednesdays, and Thursdays from 10:00 AM to 2:00 PM at the Emergency Services Building, 161 Bush River Road, Farmville, VA 23901.
- Amelia: Wednesdays from 10:00 AM to 2:00 PM at the Amelia Clinic, 9101 Washington Street, Amelia Court House, VA 23002.
What Happens Next?
- You will check in at the front desk, and staff will provide you with a form to complete. If you need any language assistance, please let staff know so arrangements can be made for a professional phone interpreter, if needed. Please note: A legal guardian of children under 18 must be present for the assessment. If the parent or guardian is unable to attend, written permission may be considered, but it must be pre-approved in advance.
- You will be asked to wait in the waiting area while your information is being reviewed by CSB staff.
- After your information has been reviewed, you will then be invited to meet with CSB staff, who will review the information provided and enter it into an electronic health record
