Clients are seen on a first come first served basis and space is limited. You are not guaranteed to see an evaluator if others have arrived before you.

The entire process can take up to 3 hours, and you may be asked to come back later that day or on a different day.

What to Bring with You?

  • ID
  • Insurance Cards
  • Social Security Card (optional)
  • Proof of income (if uninsured)
  • Referral (if applicable)

Please note: A legal guardian of children under 18 must be present for the assessment. If the parent or guardian is unable to attend, written permission may be considered, but it must be pre-approved in advance. Any legal guardianship or custody paperwork must be shown prior to any services being rendered.

What Happens Next?

Upon your check-in at the front desk, staff will provide you with a form to complete. After completing the form, present it at the front desk and relax in the waiting area while your information is being reviewed by CSB staff.

After the completed form has been reviewed, you will meet with CSB Intake staff, who will discuss with you the information provided and enter it into an electronic health record. You will then be asked to return to the lobby to await registration prior to your evaluation with a CSB evaluator.

The goal of the evaluation is to make a good assessment of the needs you have and then determine how those needs will be met. You will either be connected to medication, case management, therapy, or other services at Crossroads or it may be determined that providers in the community would be a better fit for you. Either way, we will connect you to the services you need to start your journey.
